I have added an example nginx configuration file and updated the INSTALL file with instructions on how to setup Mythweb using nginx.

The index isn't used, as everything that matches *.php ends up getting passed through to php-fpm and essentially everything gets rewritten to mythweb.php something. I can change it if there is a better pattern than what I am currently doing.
As far as passing the hostname I think you are correct that it is not required. I'll check later and update the pull request.

Great stuff but shouldn't the index be set to mythweb.php? The project has no index.html or index.htm. Also, is it necessary to pass the host name? I don't do that on my setup.
