MythNetVision channel icon theming

The channel icons should be theme-able. Having it look in a theme folder first would be a good approach; maybe have the icons provided by the plugin sit in a folder in the "default" theme.

As it is right now a themer can't really create channel icons that work with the theme's aesthetic unless they overwrite the default one provided by the plugin. This could be a problem in a couple ways. First, the last theme to be installed would end up providing the channel icons even if the theme isn't being used. Second, this could cause packaging problems since theme packages would be overwriting the same files used by multiple packages (other themes and the plugin package).
