Failed to initialize a/v sync

I am seeing an increased number of occurrences of this error when trying to view recorded media. It begin a few weeks ago when I upgraded to 0.26, perhaps once every few days. More recently it occurs several time a day in the span of a few hours recordings

I have seen this issue on my setup trying VDPAU using the latest 0.27 build.

+1 on the .27 occurrence (FC19). I have to re-initialize X to recover.

Can you supply a sample video that has the error?

I too have seen this happening at regular interval since upgrading to 14.04 which upgraded the nvidia drivers.
