Mythfrontend should be integrated and stay synchronized with ALSA mixer level changes performed externally by other applications.

After investigation, and considering many of the media packages do not synchronize with the mixer level if changed externally. I won't be implementing this feature.
While the mixer is opened, you can't read the external volume level and reading the currently opened mixer do not pass the new volume set externally.
